Research question

Across the 53 CMS reporting jurisdictions, which states' skilled-nursing-facility portfolios have accumulated the largest aggregate fine dollar volume, and how does the per-facility fine ratio compare across states with similar facility counts?

Methodology

This ranking reflects the data currently in our database, sourced from the agency referenced in the citation below and updated automatically as new filings are processed.

Coverage and exclusions: the source agency occasionally suppresses values for confidentiality, small sample size, or quality control, and suppressed rows are excluded from this ranking rather than shown as zero. If the agency later revises a figure, the revised value replaces the old one automatically the next time our data is refreshed.

Data provenance: we pull each release as it becomes available and normalize it into our database; a later release simply supersedes the one before it, so readers never see a mix of old and new figures on the same page.

Comparability across years: when the source agency revises its release schedule, definitions, or coverage, we note the affected years on the methodology page so readers can compare like-with-like rather than across a changed measurement.

What this analysis cannot tell us

Fines reflect Civil Monetary Penalties (CMPs) assessed by CMS regional offices through the Nursing Home Compare enforcement program and recorded in the public penalties dataset. Aggregate fine volume is influenced by the number of skilled-nursing facilities in a state, the size of those facilities, the historical enforcement intensity of CMS regional offices, and state-survey-agency referral rates, not solely by facility-level care quality. Per-facility fine averages divide total fine dollars by total facility count within a state and are not weighted by bed count, occupancy, or facility age. Fine dollars are nominal amounts recorded in the dataset and have not been adjusted for inflation across the multi-year reporting window. A facility appearing in the count has accumulated at least one fine event in the dataset and the figure does not separate fines later vacated on appeal from those held final. Per-facility-fine ranking applies a 50-SNF floor to avoid distortion from small-jurisdiction denominators. Enforcement activity is not equivalent to a measure of clinical quality.
