When CMS Nursing Home Compare refreshes land, methodology shifts, or we fix something we published wrong, we log it here. Each entry names what changed, which pages it touched, and the deploy or ETL evidence. This record starts in August 2026; we are not reconstructing earlier changes from memory.

1. 2026-09-04 (deploy bba16dd)

   Facility and watchlist RSS feeds for saved nursing homes

   Scope: Per-facility /facilities/{slug}/feed.xml plus /feed/watchlist.xml; links from facility hero and /saved

   Readers who save facilities can subscribe to CMS-driven processing_date updates without inventing churn. Feeds only emit when the underlying CMS extract carries a real processing date.

2. 2026-09-03 (deploy af655d6)

   State hub pages: tercile registry desks and chrome cut

   Scope: All /state/* nursing-home hubs (50 states + DC + territories with CMS coverage)

   State pages now open with a rating-band × star-shape desk ordered by deviation from the national mix, plus ownership branches. Identical CiteThis/PageTrust chrome that previously duplicated across states was cut so each hub carries reachable unique mass. Measured local rendered-similarity for the class moved from REJECT-band 85.2% to WARN 70.2%.

3. 2026-08-31 (deploy ab59763)

   Closing takeaways band on every facility detail page

   Scope: ~14,690 /facilities/{slug} entity pages

   Each facility page now ends with a TakeawaysBand summarizing the CMS figures already shown above (stars, staffing, deficiencies, penalties) so the page closes on the decision-relevant facts rather than trailing chrome.

4. 2026-08-30 (deploy 772c642)

   Layered rich-result schema on homepage and tools

   Scope: Homepage @graph plus WebApplication markup on /tools/*

   Added a layered schema stack so search and answer engines can identify the Organization, Dataset, and tool surfaces without inventing authors or awards.

5. 2026-08-21 (deploy 7ae9fc7)

   Penalty volume year-over-year ranking board

   Scope: /rankings/penalty-volume-changes (2024→2025 CMS Civil Money Penalty totals)

   Published a nationwide board that ranks states by change in CMS penalty volume between the prior and current civil-money-penalty windows, computed from the official PDC fine records.

6. 2026-08-21 (deploy 8cd4da3)

   Nationwide bed-size and staffing peer lattices on facility pages

   Scope: Facility detail Related / peer sections across the CMS certified nursing-home corpus

   Facility pages now surface peer sets by bed-size band and staffing intensity so readers can compare a home to similarly sized peers, not only to geographic neighbors.

7. 2026-08-20 (deploy f9b4714)

   Parametric compare URLs noindexed (URL bleed)

   Scope: Any compare URL carrying filter/query parameters that previously risked indexation

   Query-shaped compare surfaces remain reachable for readers but now send noindex,follow so crawlers do not treat infinite filter combinations as separate index entries.
